Last Will & Testament
Distributes your property, establishes care for your children and otherwise expresses your wishes upon your death. If you pass away without a Will, a Judge will determine how your property is distributed and who cares for your children and pets.
Health Care Directive
Appoints a health care agent who will have the legal authority to make health care decisions if a person is no longer able to make decisions due to an illness or incapacity.
Durable Power of Attorney
Appoints a person to make financial decisions for you in case you become mentally incapacitated and unable to handle matters on your own.
HIPAA Authorization
Gives permission for an individual to access your medical records in order to make the best health care decisions for you.
Living Will
Written instructions that specify what actions should be taken for health care, if you are no longer able to make decisions due to an illness or incapacity.
